    Folic Acid Supplementation Improves Vascular Function in Professional Dancers With Endothelial Dysfunction

    Objective To determine if folic acid supplementation improves vascular function (brachial artery flow-mediated dilation [FMD]) in professional dancers with known endothelial dysfunction. Design Prospective cross-sectional study. Setting Academic institution in the Midwestern United States. Subjects Twenty-two professional ballet dancers volunteered for this study. Main Outcome Measures Subjects completed a 3-day food record to determine caloric and micronutrient intake. Menstrual status was determined by interview and questionnaire. Endothelial function was determined as flow-induced vasodilation measured by high-frequency ultrasound of the brachial artery. A change in brachial diameter of Results Sixty-four percent of dancers (n = 14) had abnormal brachial artery FMD (P\u3c .0001). Conclusions This study reveals that vascular endothelial function improves in dancers after supplementation with folic acid (10 mg/day) for at least 4 weeks. This finding may have clinically important implications for future cardiovascular disease risk prevention

    Association Between the Female Athlete Triad and Endothelial Dysfunction in Dancers

    Objective: To determine the prevalence of the 3 components of the female athlete triad [disordered eating, menstrual dysfunction, low bone mineral density (BMD)] and their relationships with brachial artery flow-mediated dilation in professional dancers. Design: Prospective study. Setting: Academic institution in the Midwest. Participants: Twenty-two professional ballet dancers volunteered for this study. Interventions: The prevalence of the female athlete triad and its relationship to endothelial dysfunction. Main Outcome Measures: Subjects completed questionnaires to assess disordered eating and menstrual status/history. They also completed a 3-day food record and wore an accelerometer for 3 days to determine energy availability. Serum baseline thyrotropin, prolactin, and hormonal concentrations were obtained. Bone mineral density and body composition were measured with a GE Lunar Prodigy dual-energy X-ray absorptiometry. Endothelial function was determined as flow-mediated vasodilation measured by high-frequency ultrasound in the brachial artery. An increase in brachial diameter Results: Seventeen dancers (77%) had evidence of low/negative energy availability. Thirty-two percent had disordered eating (EDE-Q score). Thirty-six percent had menstrual dysfunction and 14% were currently using hormone contraception. Twenty-three percent had evidence of low bone density (Z-score \u3c -1.0). Sixty-four percent had abnormal brachial artery flow-mediated dilation ( Conclusions: Endothelial dysfunction was correlated with reduced BMD, menstrual dysfunction, and low serum estrogen. These findings may have profound implications for cardiovascular and bone health in professional women dancers

    Two-week joint mobilization intervention improves self-reported function, range of motion, and dynamic balance in those with chronic ankle instability

    We examined the effect of a 2-week anterior-to-posterior ankle joint mobilization intervention on weight-bearing dorsiflexion range of motion (ROM), dynamic balance, and self-reported function in subjects with chronic ankle instability (CAI). In this prospective cohort study, subjects received six Maitland Grade III anterior-to-posterior joint mobilization treatments over 2 weeks. Weightbearing dorsiflexion ROM, the anterior, posteromedial, and posterolateral reach directions of the Star Excursion Balance Test (SEBT), and self-reported function on the Foot and Ankle Ability Measure (FAAM) were assessed 1 week before the intervention (baseline), prior to the first treatment (pre-intervention), 24–48 h following the final treatment (post-intervention), and 1 week later (1-week follow-up) in 12 adults (6 males and 6 females) with CAI. The results indicate that dorsiflexion ROM, reach distance in all directions of the SEBT, and the FAAM improved (p < 0.05 for all) in all measures following the intervention compared to those prior to the intervention. No differences were observed in any assessments between the baseline and pre-intervention measures or between the postintervention and 1-week follow-up measures (p > 0.05). These results indicate that the joint mobilization intervention that targeted posterior talar glide was able to improve measures of function in adults with CAI for at least 1 week

    How can diagnostic assessment programs be implemented to enhance inter-professional collaborative care for cancer?

    BackgroundInter-professional collaborative care (ICC) for cancer leads to multiple system, organizational, professional, and patient benefits, but is limited by numerous challenges. Empirical research on interventions that promote or enable ICC is sparse so guidance on how to achieve ICC is lacking. Research shows that ICC for diagnosis could be improved. Diagnostic assessment programs (DAPs) appear to be a promising model for enabling ICC. The purpose of this study was to explore how DAP structure and function enable ICC, and whether that may be associated with organizational and clinical outcomes.MethodsA case study approach will be used to explore ICC among eight DAPs that vary by type of cancer (lung, breast), academic status, and geographic region. To describe DAP function and outcomes, and gather information that will enable costing, recommendations expressed in DAP standards and clinical guidelines will be assessed through retrospective observational study. Data will be acquired from databases maintained by participating DAPs and the provincial cancer agency, and confirmed by and supplemented with review of medical records. We will conduct a pilot study to explore the feasibility of estimating the incremental cost-effectiveness ratio using person-level data from medical records and other sources. Interviews will be conducted with health professionals, staff, and referring physicians from each DAP to learn about barriers and facilitators of ICC. Qualitative methods based on a grounded approach will be used to guide sampling, data collection and analysis.DiscussionFindings may reveal opportunities for unique structures, interventions or tools that enable ICC that could be developed, implemented, and evaluated through future research. This information will serve as a formative needs assessment to identify the nature of ongoing or required improvements, which can be directly used by our decision maker collaborators, and as a framework by policy makers, cancer system managers, and DAP managers elsewhere to strategically plan for and implement diagnostic cancer services

    GEANT4 Simulation of a Cosmic Ray Muon Tomography System with Micro-Pattern Gas Detectors for the Detection of High-Z Materials

    Muon Tomography (MT) based on the measurement of multiple scattering of atmospheric cosmic ray muons traversing shipping containers is a promising candidate for identifying threatening high-Z materials. Since position-sensitive detectors with high spatial resolution should be particularly suited for tracking muons in an MT application, we propose to use compact micro-pattern gas detectors, such as Gas Electron Multipliers (GEMs), for muon tomography. We present a detailed GEANT4 simulation of a GEM-based MT station for various scenarios of threat material detection. Cosmic ray muon tracks crossing the material are reconstructed with a Point-Of-Closest-Approach algorithm to form 3D tomographic images of the target material. We investigate acceptance, Z-discrimination capability, effects of placement of high-Z material and shielding materials inside the cargo, and detector resolution effects for such a MT station.Comment: 9 pages, 10 figures, submitted to conference proceedings of SORMA West 08, Berkele

    Real world costs and cost-effectiveness of Rituximab for diffuse large B-cell lymphoma patients: a population-based analysis.

    BackgroundCurrent treatment of diffuse-large-B-cell lymphoma (DLBCL) includes rituximab, an expensive drug, combined with cyclophosphamide, doxorubicin, vincristine, and prednisone (CHOP) chemotherapy. Economic models have predicted rituximab plus CHOP (RCHOP) to be a cost-effective alternative to CHOP alone as first-line treatment of DLBCL, but it remains unclear what its real-world costs and cost-effectiveness are in routine clinical practice.MethodsWe performed a population-based retrospective cohort study from 1997 to 2007, using linked administrative databases in Ontario, Canada, to evaluate the costs and cost-effectiveness of RCHOP compared to CHOP alone. A historical control cohort (n = 1,099) with DLBCL who received CHOP before rituximab approval was hard-matched on age and treatment intensity and then propensity-score matched on sex, comorbidity, and histology to 1,099 RCHOP patients. All costs and outcomes were adjusted for censoring using the inverse probability weighting method. The main outcome measure was incremental cost per life-year gained (LYG).ResultsRituximab was associated with a life expectancy increase of 3.2 months over 5 years at an additional cost of 16,298,correspondingtoanincrementalcosteffectivenessratioof16,298, corresponding to an incremental cost-effectiveness ratio of 61,984 (95% CI 34,08734,087-135,890) per LYG. The probability of being cost-effective was 90% if the willingness-to-pay threshold was 100,000/LYG.Thecosteffectivenessratiowasmostfavourableforpatientslessthan60yearsold(100,000/LYG. The cost-effectiveness ratio was most favourable for patients less than 60 years old (31,800/LYG) but increased to 80,600/LYGforpatients6079yearsoldand80,600/LYG for patients 60-79 years old and 110,100/LYG for patients ≥ 80 years old. We found that post-market survival benefits of rituximab are similar to or lower than those reported in clinical trials, while the costs, incremental costs and cost-effectiveness ratios are higher than in published economic models and differ by age.ConclusionsOur results showed that the addition of rituximab to standard CHOP chemotherapy was associated with improvement in survival but at a higher cost, and was potentially cost-effective by standard thresholds for patients &lt;60 years old. However, cost-effectiveness decreased significantly with age, suggesting that rituximab may be not as economically attractive in the very elderly on average. This has important clinical implications regarding age-related use and funding decisions on this drug

    Lifted temperature minimum during the atmospheric evening transition

    Observations of lifted temperature minimum (LTM) profiles in the nocturnal boundary layer were first reported in 1932. It was defined by the existence of a temperature minimum some centimetres above the ground. During the following decades, several research studies analysed this phenomenon verifying its existence and postulating different hypotheses about its origin. The aim of this work is to study the existence and characteristics of LTM during the evening transition by using observations obtained during the Boundary Layer Late Afternoon and Sunset Turbulence (BLLAST) campaign. Data obtained from two masts instrumented with thermocouples and wind sensors at different heights close to the ground and a mast with radiometers are used to study the role of mechanical turbulence and radiation in LTM development. The study shows that LTM can be detected under calm conditions during the day–night transition, several hours earlier than reported in previous work. These conditions are fulfilled under weak synoptic forcing when the local flow shifts associated with a mountain–plain circulation in relatively complex orography. Under these special conditions, turbulence becomes a crucial parameter in determining the ideal conditions for observing LTM.
